Short description

A testosterone gel that is applied topically to the skin is sold under the brand name Androgel. It is used to treat hypogonadism, or men with low testosterone levels. A hormone called testosterone is necessary for the growth and maintenance of male traits like muscle mass, bone density, and body hair.
Androgel is only available on prescription and is typically applied once per day to the shoulders or upper arms. A healthcare professional will determine the individual’s dosage and treatment duration based on their particular requirements.
It is essential to discuss any existing medical conditions or medications with a healthcare provider prior to beginning Androgel treatment. Men who have breast or prostate cancer, a high red blood cell count, or a history of blood clots may not benefit from the gel. Men who have sleep apnea, a high or low red blood cell count, or a history of blood clots should also use the gel with caution.
Acne, breast enlargement, and skin irritation at the application site are all common Androgel side effects. An increase in the number of red blood cells, alterations in mood, and difficulty falling asleep are among the less frequent adverse effects. It is essential to inform a healthcare provider if any of these side effects become severe or bothersome.
According to a doctor’s recommendation, men with low testosterone levels should take Androgel. It is not recommended for use in children or women.

Indications for use of Androgel

Replacement therapy for endogenous testosterone deficiency.
Interaction with other drugs
Androgel® should be administered with caution with oral anticoagulants, since it is possible to enhance the effect of oral anticoagulants by modifying the synthesis of hepatic coagulation factor and competitive inhibition of plasma protein binding. It is recommended to control prothrombin time. Patients receiving oral anticoagulants require frequent monitoring, especially at the beginning and/or end of androgen treatment.

Coadministration of testosterone and ACTH or corticosteroids may increase the risk of edema. These drugs should be used together with caution, especially in patients with heart, kidney or liver disease.

Effects on laboratory tests: Androgens can decrease thyroxin-binding globulin levels, resulting in a decrease in serum T4 concentrations and an increase in sensitivity to T3 and T4. Free thyroid hormone levels, however, remain unchanged, and there are no clinical manifestations of hypothyroidism.
Cases of overdose with the use of Androgel have not been recorded. Only one case of overdose has been described following the use of injectable testosterone. It was a stroke in a patient with a high plasma testosterone concentration of 114 ng/mL (395 nmol/L). However, it is unlikely that similar plasma testosterone concentrations can be achieved when the drug is applied to the skin.

pharmachologic action

Pharmacological group:

Pharmacological properties:

– Androgel® creates and maintains a stable level of testosterone within the physiological norm, starting from the second day of therapy (for comparison, the equilibrium concentrations of testosterone when used in injection form are established only between the 3rd and 5th injections, that is, almost a year after initiation of therapy). – When using Androgel, there are no undesirable supraphysiological peaks in testosterone concentration, which inevitably occur when prescribing injectable drugs. This is reflected in the instructions for the drugs. Periods of supraphysiological and hypogonadal testosterone concentrations created by injectable preparations increase the risk of adverse events (increased hematocrit, priapism, attacks of aggression, cardiovascular complications). – If necessary, Androgel® can be quickly canceled. After the withdrawal of Androgel, the serum testosterone concentration returns to the values that existed before the start of treatment, within a maximum of 4 days after the last application. (For comparison, the half-life of prolonged injectable drugs reaches 90 days.).

Endogenous androgens (mainly testosterone) secreted by the testicles and their main metabolite dihydrotestosterone are responsible for the development of the external and internal genital organs and for the development and maintenance of secondary sexual characteristics (stimulation of hair growth, coarsening of the voice), libido; for the overall effect on protein anabolism; for the development of skeletal muscles and the distribution of subcutaneous fat; for a decrease in urinary excretion of nitrogen, sodium, potassium, chlorides, phosphates and water. Testosterone does not cause testicular development: it reduces pituitary secretion of gonadotropins. The effects of testosterone on some target organs appear after the peripheral conversion of testosterone to estradiol, which then binds to estrogen receptors in the nuclei of target organ cells (such as the pituitary gland, adipose tissue, brain, bones, etc.).

The degree of absorption of testosterone through the skin varies from approximately 9% to 14% of the applied dose. After absorption through the skin, testosterone enters the systemic circulation in relatively constant concentrations for 24 hours. Serum testosterone concentration increases from the first hour after the use of Androgel®, reaching a constant value from the second day of treatment. Daily fluctuations in testosterone concentrations have the same amplitude as the changes in the content of endogenous testosterone observed in circadian rhythms. With the external route of administration of the drug, there are no supraphysiological peaks in the concentration of testosterone in the blood that occur during the injection method of application. In contrast to oral androgen therapy, topical application of the drug does not cause an increase in steroid concentrations in the liver above physiological norms. The use of 5 g of Androgel causes an average increase in plasma testosterone concentration of approximately 2.5 ng / ml (8.7 nmol / l). After stopping treatment, testosterone levels begin to decline approximately 24 hours after the last dose. The concentration returns to baseline approximately 72-96 hours after the last dose. The main active metabolites of testosterone are dihydrotestosterone and estradiol. Androgel® is excreted mainly in the urine and feces as conjugated testosterone metabolites.
Pregnancy and lactation
Pregnant women should avoid any contact with the drug due to its possible virilizing effect on the fetus. In case of contact with the drug, wash the contact area with soap as soon as possible.

Side effects

The most common adverse effects (about 10%) when using the recommended dose of 5 g of gel per day were: skin reactions at the site of application, erythema, acne, dry skin. During clinical trials of Androgel, the following undesirable effects were noted (> 1/100, <1/10):

On the part of the blood and lymphatic system: changes in the results of laboratory tests (polycythemia, changes in lipid levels).

From the genitourinary system: changes in the prostate gland, gynecomastia, mastodynia.

From the nervous system: dizziness, paresthesia, amnesia, hyperesthesia, mood changes.

From the side of the cardiovascular system: increased blood pressure.

From the gastrointestinal tract: diarrhea.

From the skin and its appendages: alopecia, urticaria.

General disorders: headache. The drug contains alcohol, so if it is often applied to the skin, irritation and dryness may occur.

special instructions

Androgel® should be used only in case of testosterone deficiency, accompanied by such clinical manifestations as: underdevelopment or regression of secondary sexual characteristics, changes in body structure, carbohydrate and lipid metabolism disorders, obesity, asthenia, sexual dysfunction (decreased libido, erectile dysfunction, etc.) , decreased bone mineral density, mood swings, depression, hot flashes, etc. Before starting treatment, other possible causes underlying the above symptoms should be excluded. Currently, there are no clear age norms for testosterone levels. However, it must be taken into account that the physiological levels of serum testosterone begin to decline from the age of 30-40, and the level of sex steroid-binding globulin increases. This, accordingly, leads to a decrease in the level of biologically active testosterone.

Due to the variability of laboratory values, the determination of testosterone concentration should be carried out in the same laboratory.

Androgel® is not used to treat male infertility or erectile dysfunction, the cause of which is not related to testosterone deficiency.

Before testosterone is prescribed, all patients should be screened to exclude the risk of prostate cancer, as androgens can accelerate the progression of subclinical prostate cancer and benign prostatic hyperplasia. Careful and regular monitoring of the prostate gland (digital rectal examination, determination of prostate-specific antigen (PSA) in serum) and mammary glands should be carried out at least once a year, and in elderly patients and patients at risk (with clinical or family factors) – twice a year.

Testosterone preparations should be used with caution in patients with malignant neoplasms because of the risk of hypercalcemia (and concomitant hypercalciuria) due to bone metastases. In these patients, it is recommended to monitor the concentration of calcium in the serum.

In patients with severe cardiac, hepatic or renal insufficiency, treatment with testosterone preparations may cause complications characterized by edema with or without congestive heart failure. In this case, treatment should be stopped immediately. In addition, diuretic therapy may be required. In patients taking androgens for a long period, in addition to laboratory measurements of testosterone concentration, the following laboratory parameters should be periodically checked: hemoglobin, hematocrit (to detect polycythemia), liver function tests and lipid profile.

Data have been published on an increased risk of sleep apnea in hypogonadal patients treated with testosterone esters, especially those with risk factors such as obesity and chronic respiratory disease.

In patients with diabetes mellitus receiving androgens, when normal plasma testosterone concentrations are reached, an increase in insulin sensitivity may be observed.

Some clinical symptoms: irritability, nervousness, weight gain, prolonged or frequent erections may indicate excessive androgen exposure, requiring dosage adjustment. If a patient develops a severe local reaction, treatment should be reviewed and, if necessary, discontinued. When using Androgel in athletes, it is necessary to take into account the fact that this drug contains an active substance (testosterone), which can give a positive reaction in anti-doping tests.

Influence on the ability to drive a car and other mechanisms

At present, there is no data on the effect of Androgel on the ability to drive a car and on the control of machines and mechanisms.
Storage conditions
At a temperature not higher than 25°C.

Keep out of the reach of children.

Dosage and administration of Androgel

The recommended dose is 5 g of gel (i.e. 50 mg of testosterone) applied once a day at about the same time, preferably in the morning. The individual daily dose may be adjusted by the physician depending on clinical and laboratory parameters in patients, but should not exceed 10 g of gel per day. Correction of the dosing regimen should be carried out in steps of 2.5 g of gel per day.

The gel is applied to clean, dry, intact skin of the shoulders, upper arms and / or abdomen. When using the drug in a vial, in order to obtain a full-fledged first extractable dose, it is necessary to fill the container of the dosing pump. To do this, hold the container vertically, slowly and completely press the dosing pump 5 times. After the first five presses, discard the resulting gel, taking precautions. Filling the container of the dosing pump is necessary only before receiving the first dose to be taken. After the procedure for filling the container of the dosing pump, it is necessary to press the dosing pump 1 time to release 1 extractable dose of 1.25 g of gel containing 12.5 mg of testosterone.

The gel should be removed from the bottle or from the package directly into the palm of your hand, and then applied to the required application sites. After opening the package, it is necessary to immediately apply all its contents to the skin and distribute in a thin layer.

It is not necessary to rub Androgel® into the skin. Allow the gel to dry for at least 3-5 minutes before dressing. Wash hands with soap after application. Do not apply the gel to the genital area, as the high content of ethyl alcohol in the preparation may cause local irritation.

A constant concentration of testosterone in plasma is reached approximately on the second day of treatment with Androgel®. To adjust the dose of testosterone, it is necessary to determine the concentration of testosterone in serum in the morning, before using the drug, from the third day after the start of treatment (within one week). Dosage may be reduced if plasma testosterone levels are elevated. If the concentrations are low, the dose can be increased, but not more than 10 g of gel per day.

F.A.Q. related to Androgel

What is androgel?

Androgel is a medication that contains testosterone and is used to treat low testosterone levels in men, also known as hypogonadism. This is a gel that is applied topically to the skin, usually on the shoulder or upper arm, and is absorbed through the skin into the bloodstream.

How long does it take for androgel to work?

Depending on the individual, the effectiveness of Androgel may take a different amount of time. Some men may experience significant improvement in their symptoms within a few days, while others may experience significant improvement over several weeks or even months.

It is critical to adhere to the dosing guidelines given by your medical services supplier and use Androgel reliably and as coordinated to accomplish the best outcomes. To ensure that you are receiving the appropriate amount of testosterone replacement therapy, your healthcare provider may also monitor your testosterone levels and, if necessary, adjust your dosage.

Where to apply androgel for maximum absorption?

It is essential to apply it correctly in order to maximize its absorption. Some suggestions:

  • Use Androgel on dry, clean skin. It should not be used on broken or irritated skin.
  • Apply Androgel to the stomach, thighs, shoulders, and upper arms. It should not be used on the genitalia.
  • To prevent skin irritation, rotate the application site each time.
  • Before you put on clothes, take a bath, or go swimming, let the gel completely dry.
  • After applying Androgel, thoroughly wash your hands with soap and water to prevent accidental transfer to others.
  • Androgel should not be applied more than once to the same area in a 24-hour period.
  • Based on your specific requirements and medical history, discuss specific application instructions for Androgel with your healthcare provider to ensure maximum absorption.

How to apply androgel?

AndroGel application steps are as follows:

  • Use soap and water to thoroughly wash your hands.
  • Prime the AndroGel pump by pressing down on it three times after removing the cap.
  • Place the gel on dry, clean skin. Avoid using on the genitalia or other delicate areas.
  • Before dressing, evenly distribute the gel over the application area and allow it to dry for a few minutes.
  • Clean up with cleanser and water subsequent to applying the gel.
  • After applying the gel, do not take a bath, swim, or shower for at least two hours.
  • AndroGel should be applied every day at the same time, preferably in the morning.
  • Apply only the recommended amount of AndroGel.

AndroGel should be used according to your doctor’s instructions because it can cause side effects or reduce the medication’s effectiveness.

Will androgel make me bigger?

Androgel is not a magic bullet for muscle growth, but it can help you maintain a healthy body composition and improve your physical performance. You will need to work out regularly and eat a healthy diet that is high in protein and other nutrients that help muscle growth if you want to get bigger.

Androgel, like any other testosterone therapy, should only be used under the supervision of a trained medical professional. Abuse of these drugs can prompt serious unexpected problems, including coronary illness, stroke, and prostate disease.

Where to buy androgel online?

If you are considering buying Androgel online, it is important to be careful and only buy from trusted sources. It is also important that you consult a medical professional to ensure that testosterone gel is appropriate for your particular medical condition and to determine the appropriate dosage.

Can I put androgel on my balls?

Applying AndroGel directly on the scrotum or testicles can cause irritation, discomfort, and even damage to the delicate skin in that area. Additionally, the skin on the scrotum is not as absorbent as the skin on other parts of the body, so the medication may not be as effective if applied there.
